Here are the steps to follow to ensure we can track participation by the PS261 Community:
Taking The Survey By Hand:
1. IF YOU HAVE ALREADY FILLED OUT THE SURVEY BY HAND, AND MAILED IT BACK:
Use the form below to email your ELDEST Child's name & class, so that we can track family participation in the survey.
2. IF YOU HAVE ALREADY FILLED OUT THE SURVEY BY HAND, AND NOT YET MAILED IT BACK:
PLEASE return your completed survey, inside the included return envelope, to the Main Office - someone will take down your ELDEST CHILD's name & class, so that we can track family participation.
Taking The Survey Online:
3. IF YOU HAVE ALREADY TAKEN THE SURVEY ONLINE:
PLEASE EMAIL survey@ps261.org with your ELDEST Child's name & class so that we can track family participation - use the form below.
4. IF YOU INTEND TO TAKE THE SURVEY ONLINE:
PLEASE fill out the information in the form below, and click the button to be redirected to the DOE Online Learning Environment Survey.
