Parents -
School Lunch forms have gone home in backpacks - they are red and black NYC
Department of Education forms. Please make sure to fill out this form and
return it to your child's teacher.

Just fill out ONE form per family. Follow the instructions at the beginning of
the form. EVEN IF you feel you do not qualify for free or reduce-priced meals,
please fill out the form.

Rest assured - no child will be stopped from getting on line and having lunch at
any time. After all forms are processed (the forms are sent outside the school
for processing elsewhere), you will be notified about how to proceed.

Until that time, each and every child can have breakfast and lunch in the
cafeteria, at any time, at no cost. But please do fill out the form!
